To the best of my knowledge, this child is in good health, and is physically and mentally able to participate in all activities, except as noted in the space provided below.
If notice of cancellation is received between time of registration and prior to the registration close date, your are entitled to a refund of the registration fee less $25. If notice of cancellation is received on or after the close date, the registration fee will not be refunded.
The Kitchener-Waterloo Humane Society reserves the right to cancel the program if registration is low.
I understand that the Humane Education Coordinator reserves the right, at any time, to dismiss a participant at his/her sole discretion if, in his/her opinion, the participant is a hazard to others, impairs the safety or rights of others, appears to have rejected the reasonable controls of the club, and/or has failed to cooperate in any manner. If such a situation occurs, I will be requested to make arrangements to have my child removed from the program immediately.
I understand that the K-W Humane Society may use any photographs of my child taken during the this program in the future for promotional purposes (no names will be published).
I understand that all information obtained will be kept confidential and that at no time will the Kitchener-Waterloo and North Waterloo Humane Society sell, lend, or give my personal information to any group or person unless required by law (To view our privacy policy in full, visit our website at www.kwhumane.com).
I, the undersigned, hereby give permission for my child or ward to attend and participate in all activities involved in the Kitchener-Waterloo Humane Society’s Education Program. I agree that they will abide by the rules set by the Humane Society, and I will cover any costs should they occur due to my child’s negligence or mischief. I further absolve the Kitchener-Waterloo Humane Society of any liability from any incident which might occur during the Education Program. I have read this form and I understand and accept the conditions of participation.
I hereby certify that all the information in the Health section is complete, accurate, and up to date. I will contact the Kitchener-Waterloo Humane Society, in writing, if any changes occur in the child’s health status between now and the start of the session.
